Tested the save. First of all the save is amazing and huge thanks to @MightyQuack for providing it. This save is a true god mode for the DLC.

Once you load the save the game drops your health to 1/10 of the real value. You will need to suicide your character once by doing a health upgrade and then letting him to be killed.

Once the game loads you back in the main area or whatever, you will have full health with God mode. The save is already set to Master difficulty with the highest reward bonus.

I am far into the DLC on master difficulty using this save and so far the game hasn't removed the god mode.

I would suggest not do scarifies health for rewards for you get to that point and don't mess around with your health upgrades and bonuses much.
