The last collectible is on the "Infernal Fantasy Map." First, regionalize your Secrets save data, put the game in sandbox mode, and press the triangle button to go behind the chained statue. There you'll see a sword; stand in front of it, and that's the last collectible.
Also, by enabling mod access in the options menu, you can find quite a few trophies by searching for them.
