All these saves are at the last boss with the Demon Costume equipped for 2x Damage
After you complete one of the difficulties you can start a NG+ for that same difficulty to try and pop other trophies

Demon Hunter Complete all chapters as Demon Hunter

40.46% Uncommon

Trophy pops when Johnson is singing after the ending cutscene (which you can skip, thankfully)
I got all other trophies with this save so maybe you can pop some trophies killing the 1 mob the last boss spawns in different ways/weapons

High in Las Vegas Upgrade everything availabe in the game.
