This is a fun game to play, but creating this save set was a bit frustrating since manual saving isn't possible. It took several hours, but I finally got everything sorted (I hope!).
I've tried to explain everything as simply as possible, and some folders even include screenshots for guidance. You'll need to play through certain sections as described, as the game doesn't allow manual saves. It autosaves at some odd points, and sometimes not at all.
There are 23 different folders containing save files. It ended up being quite a large save file, and you'll need to re-encode both files in each folder for them to work correctly.

001-Out.of.ammo__Childhood.friend:
When you load the save file and select "Continue," simply press Triangle (Trophy: Out of ammo), and then X.
After that, exit the room to your left. At the counter, go right into the hallway, then left. Inside that room, you'll find a globe (I'm not sure if you need to click on it, but I did while he was talking, and then I got the "Childhood Friend" trophy).
002-Captain.of.the.Sea: (Wait approximately 15-20 minutes).
Simply click on the bells on the right and left sides to avoid the mines. Once you're through, just wait for the trophy to pop.
003-Top.Floor.Please: (Wait approximately 10 minutes).
Go straight ahead, then down the stairs to the left. Follow the path until you reach a room, take the elevator up, and you'll get the trophy.
004-Hello.Kitty__Pen.Pal: (Wait about 20 minutes).
You can skip the video. Afterwards, choose Harry. Follow Barrett until he turns right. A cat will appear there; press Triangle then X to take a picture of it. Continue in the same direction as Barrett until you see the postman standing with a sack. Talk to him, and you'll receive a letter. You'll then send a letter back with a picture, and you'll earn the "Pen Pal" trophy.
005-Health.Warning: (Wait for about 10 minutes).
Follow Jas, who will be talking to you, until you reach the "house" on the left side (where Barrett is inside). On the right side, you'll see the postman. Inside the house, there's a pack of cigarettes—take them. Go back the way you came, then turn left and walk forward until you see a man smoking. Give him the cigarettes.
006-My.Saviour: (Wait for about 20 minutes).
You'll need to play through a bit to reach the next trophy. Follow Barrett, but be careful not to get shot. Wait for the enemy to reload, then sprint (hold R2) towards Barrett. If you need to crouch, press L2. At the next cover point, stop and wait. Barrett will run ahead, but you should stay put. Barrett will try to shoot the enemy; when he does, press X.
007-Hide.and.Seek__Invincible__Part.I__Puurrfect:
This part should have been included with the previous save file, but I forgot to pick up some collectibles, which is why it's separate now.
From where you start, pick up the "letters" to your right. Then, go straight into the tunnel, but don't go too far in. Inside the tunnel, there's an entrance to the left; you'll find another "letters" there.
Go back out and continue straight through the tunnel. Climb down the ladder and keep going forward. Press Circle and then X to get the Trophy for Chapter 1.
Now you'll control the cat. When you come across two soldiers, press X in front of them.
008-And.Your.Enemies.Closer...:
I've included a picture that shows you how to solve the puzzle. You don't need to worry about collecting any letters. Just go up to the red light and press X. The cables should be connected to form two 'X' shapes.
Rescue Harry, then go to the right side where there's a passage. You'll need to lift a wooden beam there. Then, switch to Harry and take a picture of Kurt. To take the picture, press Triangle, then X.
009-Unforgettable.Luncheon__The.Pied.Piper:
You'll be playing as the cat again. Just follow the bird and press X to jump towards it. You'll get the trophy when you enter a room full of food.
After the cutscene, when you're playing as Harry, you'll need to collect some letters – if I understand correctly, this is how I earned this trophy. There will be some pictures showing you where the letters are located. The last picture is where I got my trophy.
010-Goaaaal: (Wait for about 10 minutes).
Choose Harry and go down the street, taking the first left. You'll see a football. Go all the way down with the ball towards the goal.
011-Lest.we.forget__Feline.Therapy: (Wait for about 10 minutes).
Go up the ladder and continue straight, then turn right at the red shed. Cross the bridge and go all the way forward towards the cart that's to the right of the tree. Pull the cart back the way you came until you get the trophy.
Now you'll control the cat. Go towards the tree; you'll see a man crouching and crying. Press X to get "Feline Therapy."
If you want to, just follow the man the cat comforted, then click on the grave with a shovel in front of it to proceed to the next trophy, or you can load save file 012.
012-Dance.Revolution:
Just follow the cat until you reach a scene. Once you're done with the performance, you'll get the trophy.
013-Closure: (Wait for about 25 minutes).
There are two pictures showing you where to go and where to click on the bodies to get the trophy.
014-Fresh.air__Red.Cross__Explorer__Jammed: (Wait for about 25 minutes).
Now you'll need to play for a while. Unfortunately, you'll also need to collect some items.
You'll be playing as Harry. Go straight ahead, then turn left and click on the ladder.
Now you'll play as Kurt. Just follow this YouTube clip: (I'm not the owner of the YouTube clip).
A TIP: After you get the "Red Cross" trophy and choose to play as Kurt again, before you go up the ladder, to the left there's a device you turned on earlier. You should turn it off to get "Jammed," so you don't have to go back. And once you get the "Explorer" trophy, you don't need to play anymore.
015-So.near.and.yet.so.far__Part.II:
You're playing as Kurt; just pull Harry in. After the cutscene, you'll be playing as Harry, and you just need to easily get past all the soldiers. Your goal is to reach the door you saw in the clip. There are pictures showing you how to do this.

This is where Kurt should go and pull away the box for Harry.

At 1, Kurt should talk to the soldier, then go to point 2 to hold up the wooden beam.
016-Ludicrous.Display: (Wait for about 10 minutes).
You'll need to play a bit again now. As Kurt, I chose these answers for the letter: "Home soon, Harry, and cat."
Now that you're playing as Harry again, just follow Julian and talk to him. Next, you need to get the football down from the roof with the bird. Talk to the person who has the ball, then just press X multiple times to kick the ball away.
017-Burn.It.All: (As usual, wait a few minutes).
There's a picture showing you where to pick up the logs. Each time you pick one up, you need to put it into the stove on the right side of the girl.
018-Generous:
Just go straight ahead in the train until it comes to a complete stop. Press X to pull back the large bags, or whatever they are. To your left, there's chocolate; take it and go back to the carriage with the injured soldiers. Give the chocolate to the two soldiers on the right side. (See picture).
019-All-in__Bridge.Club: (Wait for about 10-15 minutes).
Choose Harry. When you start, just walk straight down the street until you see a fountain. To the left of it, there's a table with three women playing cards. When you're playing cards, you need to press X when the symbols on the cards match. Be quick!
020-Greengrocer__Ghost.In.The.Way__The.Sorrow: (Wait for about 25 minutes).
Outside, there's a tree on the left side. Press X to have the bird knock down the apples. Leave the apples inside the house in a bowl on the girl's west side. There are a total of four apples.
Afterwards, go back outside towards the shed. Move the wooden box and spin the wheel to open it (a picture is provided). Go inside, and you'll then play as Kurt. Run into as many soldiers as you can to get the next trophy. Then, click Options, Quit, and Yes. Click Options again, X, and Continue. Now, you need to navigate through without running into any soldiers to get the "The Sorrow" trophy.
021-Might.is.Right__Heart.of.Darkness: (Wait for about 5 minutes).
Run into the shed and click on the hot air balloon. Afterwards, you'll play as Kurt. Press Circle three times for the next trophy.
After the cutscene, choose Kurt. Just follow the path all the way up until you see the house. First, go towards the left window and press X. Then, go to the right and around the house to the next window, and press X again.
022-Howeward.Bound__Lone.Avenger__Part.III__A.Credit.To.You:
When you're playing as Barrett, press Square twice when given choices, and the last choice should be Circle. When you see Harry flying the hot air balloon, you'll hear Julia speak; choose Square.
When you're playing as Kurt again, keep pressing Square repeatedly. Press Square until you break free, then run away. Pick up the collectibles. Continue to get away from Barrett. When you get choices again, press Square again, then Circle three times. After that, just watch all the cutscenes and the credits to get the final trophy. (Do not skip the credits).
Trophy: Forgiven (No save file).
Start the game again by selecting "Continue." You should still be trying to get away from Barrett.
When you're given choices again, click Circle, Square, Circle, Square, Circle, and then Square twice. After you get the trophy, you can click Options and Quit.
023-The.Last.Trophys: (Friendship__Adventurer__Historian__Betrayal__Chekhovs.Gun__Sacrifice).
Click: Square, then Circle four times. Get yourself free by pressing Square repeatedly.
When you're playing as Harry, go forward a bit, then to the right of the rock there's a collectible; pick it up.
Continue forward. When you get choices, click Square four times. Then Circle three times. Once you've earned all the trophies, you can simply click Options and Quit.
Trophy: Sacrifice (No save file).
Just click "Continue" to start the game again. Go straight ahead until you reach Kurt and Barrett. Choose: Square six times, then Circle. After the trophy, go to Options, Quit, then "Continue" again to start the next one.
Trophy: Peace (No save file).
Go straight ahead again. Choose: Square twice, Circle three times, and then Square. After the trophy, go to Options, Quit, and then select "Continue."
Trophy: Revenge (No save file).
Same as before. Choose: Circle three times, and the rest Square. After the trophy, go to Options, Quit, then select "Continue."
Trophy: Grudge & Platinum (No save file).
Same as before. Choose: Square four times, then Circle twice, then Square twice.
