1 - A Way Home

Go up the stairs and kill the boss, after the whole scene, the trophy will pop.
6 - The Fall of Gult
You will have to follow the path through the tunnels and look for a lever on the wall, then find a kind of manhole to enter, there will be boards blocking the path, just equip the heart and shoot to break it, pick up the axe that is on the ground, kill the enemy then enter the other manhole to face the boss and get the trophy.
7 - Tower of the Gorger
Look to your right and you will see a gate and on the side there will be a lever, interact with it for the boss to appear, kill the boss then interact with the lever again and continue until you open the door.
8 - The Fall of Gron
go right and kill the boss.
9 - Underneath Death and Decay
Activate the two levers near you to open the door, enter and kill the boss and leave the place.
10 - The Fall of Blael
Go to your right and go straight ahead, turn right when you see an NPC with a sword stuck in his chest, go down the stairs on your right and continue until you reach a kind of tunnel and as soon as you turn left you will see a gated door, open the door and kill the boss.
11 - The Impending Frost of Above
Just follow the path all the way on the ice until a cutscene appears and then the trophy.

Enter the portal and kill the boss

Grab the scroll on the ground (red shiny item) and after the scene kill the boss.

Follow the path to the right, jumping over the platforms until you find a door that you must enter.
15 - The Fall of Rott & The Castle of Rott
Enter the door and kill the boss, don't try to kill him in the center of the room, as you need to collect his soul for the trophy to pop, then enter the red light to pop the trophy.
