Complete Save Set
Disclaimer: Now it is a complete save set. Sort the saves by date and look for the one you need. They should be in the same chronology as listed below.
So, in the big zip file you have exactly 24 saves (plus the main save file, no idea if it is needed but I have included it as well) that will net you 25 out of the 26 (not counting platinum) trophies. The only one missing is the one for killing less than 175 creatures.
After all of those you will find the last trophy for killing less than 175 create in a separate zip file.
Here is a complete list with all trophies in the correct order. Most of them are just before the action you need to do to get the trophy. Others are before the battle after which you get the trophy. Some of the battles (mainly those against the dragons from the DLC) are tough but still doable with that party.

What To Do: Select Medium Shield (Heater) from fifth character and apply “Fine”
5. Trophy: Completed Act I

What To Do: Go to Copperlane
6. Trophy: Chef

What To Do: Create Food: Dragon Egg Dish
7. Trophy: Disposition

What To Do: Talk to man, select first option 3 times, then chose [Diplomatic] option
8. Trophy: 5 Upgrades in Stronghold
Save Date/Name 02/02/2022 Heritage Hill
What To Do: Travel back to your Stronghold, the trophy will pop when you arrive
9. Trophy: All Upgrades in Stronghold
Save Date/Name 02/02/2022 Stormwall Gorge
What To Do: Travel somewhere for at least 13 hours and the trophy will pop when you arrive
10. Trophy: The Watcher With 8 Friends
Save Date/Name 03/02/2022 Dyrford Village
What To Do: Talk to Grieving Mother and recruit her
11. Trophy: The Storied Adventurer
Save Date/Name 03/02/2022 Anslog’s Compass
What To Do: Talk with woman on dock to the left, kill enemies and finish dialog
12. Trophy: Completed Act II
Save Date/Name 03/02/2022 Hadret House
What To Do: Interact with woman in bed and finish dialog
13. Trophy: The Heir of Caed Nua
Save Date/Name 03/02/2022 Yenwood
What To Do: Finish the battle and the conversation that follows (with Diplomatic option)

What To Do: Kill the dragon. It was an easy battle for me, so it should be OK
15. Trophy: Appease All of the Gods

What To Do: Turn in the last quest in front of you
16. Trophy: Completed Act III

What To Do: Jump into the pit
17. Trophies: Won the Game!!! and Super Murderer

What To Do: Interact with the machine to finish the game

What To Do: Enter the room and kill everyone
19. Trophy: Terror of the White March
Save Date/Name 04/02/2022 Ice Cave
What To Do: Attack the dragon and kill it. This was the hardest battle of all. Just attack the dragon directly with any magic spells you like and prey to get lucky. You need to kill the dragon and the trophy will pop. Even if the minions kill you after that.

What To Do: Interact with the Forge, place disk, win battle
21. Trophy: Explorer
Save Date/Name 04/02/2022 Whitestone Hollow
What To Do: Enter the cave in front of you
22. Trophy: Among the Moss and Peat
Save Date/Name 04/02/2022 Mowrghrek Ien
What To Do: Climb the vines and provoke enemies to fight, win fight. Focus on each dragon individually. This battles was much easier than the above one.
23. Trophy: A Voice From the Deep
Save Date/Name 04/02/2022 The Abbey of the Fallen Moon
What To Do: Walk towards the wall of water
24. Trophy: Called to their Labor
Save Date/Name 04/02/2022 Ionni Brathr Fragment
What To Do: Interact with the crystal, go through the dialogs and let them return to themselves at the end.
25. Trophy: Relative Pacifism
Save Date/Name 05/02/2022 Sun in Shadow
What To Do: Interact with the machine to finish the game
