Bloody Palace
To get All Game Modes trophy you will have to complete Bloody Palace. I could make a save with it completed and Human available but it would not change the fact you'll need a bloody palace trophy to look legit.
I think these trophies only pop if you do them again , so skip save 07 and 08, complete the BP for every char except Dante and try to get no damage on save 10

Save 07-11 have Turbo ON, turn it Off to make the game "easier"

As much as I love DMC 4, some of these were borderline boring to get.
