Find all Perks.
third area check the second screen

Conqueror
Defeat all Minibosses.
third area check the second screen

with this save, if you follow the steps you can get the trophies for
Lea'yi'elih & Zael'va'tielih Defeat Legion & Salvation,
they are in Region 3
after you defeat the main Boss you will get the second EldenFragment dont use it yet.
go to the shelter to fight the
The Crawling Chaos Defeat Nyarlathotep.
after that fight, go for
Embrace,Corrupt seven Abilities.
visit Region 1 then go back to the shelter and defeat
One with the Void Defeat Humanity.
he appears only after you Corrupt seven Abilities

this save gives you Three Trophies

burn all seven Shards to get (first area )

Science Prevails
Destroy your first Elder Shard.

Resist
Destroy seven Elder Shards.

after that teleport back to your Shelter and Upgrade your new abilities

then go for

Defeat Nyarlathotep & The Shining Trapezohedron.

walk up the wall to regenerate your shiled,

Hey Mate thx for the save. To anyone who needs the Achiever trophy. The one missing perk is the "Executioner" one. It con be found in the cathedral ( right area ). Take the elevator shortcut up to the end. And then move to the right a few rooms. You can see the symbol on the map ( blue glowing ). thx for the upload :)
