It's a modded save for individuals that wish to play the game on Death March Difficult without using an NG+ save. It has Infinite Health/ Stamina (God Mod).
Note - It might stop working if you change anything in Charactor or Change Mutation, so do it at your own risk since it can break the command.

Update:
If you wish to Mod your own save by using Console Command, follow the below highlighted process and you can google console commands.
Here's the process to transfer your save from PS to PC or PC to PS
1. Launch Witcher 3 on PS4/ PS5
2. Go to “My Rewards”
3. Make a note of the link or Use QR Code
4. Load the link on PC
5. Login in to your GOG account/ create one then login
6. Go to “My Rewards” to claim your bonus items
7. Load a Save
8. Save the game manually and then wait for Cloud sign to appear
9. Launch the game on PC
10. Load the save made on Cloud
11. Voila… you have your progress from PS4/ PS5 to PC and it can be vice a versa
