I've got these Trophies with this save:
Upscale Darling: Skin every single element of your inventory.
Vaultmarcher Regicide: Defeat Queen Amadalah in battle
Our Makers and Killers: Unlock 40 skills.
also kill some mobs for few trophies:
-Punishment Itself: Defeat 100 Specialist tier enemies with one character.
-Warming Up: Defeat 500 Underling tier enemies with one character.
Pomp and Circumstance: Defeat the Justicar in battle
-Continue in story, you'll be at the final boss from Chapter 1.
Children's Trial: Complete Chapter I.
Sink or Swim: Complete the first Chapter without dying.
-After killing the boss go to the King after the the conversation finish the Chapter 1 and you'll get the 2 trophies.

With this save you can get:
Bloodslick: Defeat 5 000 Underling tier enemies with one character.
So Said The Prince: Defeat 20 Champion tier enemies with one character.
Neophyte Collector: Loot your first unique item.
-Travel into an dungeon in Helmshire, there will be a boss, he will drop an unique item.
Shoot the Caster First: Defeat Mistshaper Malikia in battle
-After the battle next level is in ''Helmshire'' that's where the Secret boss is.
Play a little longer and you'll get also:
-Proven Initiate: Reach level 30.
-Whispers in the Concordat: Reach level 30 without dying.

Your at the end of Chapter 2 before the boss fight. Kill the boss and finish the chapter.
Stable Funds: Pick up 50 000 gold with one character.
-Got this while playing, not sure if you still can get it if so please let me know.
Closer to Genesis: Loot your first legendary item.
-You will get this randomly, from on this save there are some chests/bosses who drops Legendary items.
Gilded Extravagance: Fully equip with only legendary items.
-Equip the last weapon to earn this trophy.
Sins of the Flesh: Defeat Lambach in battle
-kill the boss
Hard Earned Allies: Complete acts one and two without dying.
Aberration Purified: Complete Chapter II.
-Finish the chapter.

The final secret boss is Big Bertha and she is found right around the corner from the waypoint named “Cell H” at the end of the battlefield section of Act 3.
Seasoned Fighter: Reach level 60.
Nightmares in the Republic: Reach level 60 without dying.
-Your almost lvl-60, keep killing untill you lvlup.

Senseless Relapse: Defeat the Final Boss of Act 3
-Your at the last level in Act 3, Kill the boss and get the trophy.
Trial By Fire: Complete acts one, two, and three without dying.
Into the Fury: Complete Chapter III.
-just Finish the Chapter 3
in beginning from chapter 4 you can easily get these trophies:
Throatslitter: Defeat 1 000 Specialist tier enemies with one character.
-Kill only few more and will popup
Primordial Excavations: Beat a dungeon in Champion of Stormfall with at least one map modifier.
Low Level Management: Complete your first mission in Champion of Stormfall.
Renovations: Construct your first building in Champion of Stormfall.

Walk Through Fire: Defeat Ahriman in battle.
Shatter the Evil: Complete the fourth act.
Flawless Victory: Complete acts one, two, three and four without dying.
Well Supplied: Unlock all tabs in your personal chest.
-You will get this after finishing few levels.
Total Ascension: Unlock all Apocalyptic Forms.
-final form is awarded by building the Anankis Artifact in endgame mode
